We were advised to arrange via our bank as it should be cheaper than if You deal directly with the health provider.

There's a few private medical insurances to choose from - DKV, ASSA are just two that you can discuss with local insurance brokers.  Price will depend on age, existing medical conditions etc..  For residencia/TIE purposes it must be with 'no co-payments'  which means no excess.  Like any insurance this makes it quite expensive.  Before Brexit kicked in at the beginning of this year, you could amend your insurance level after the first year (once you had your residencia) however, as the TIE requirements are a little different this also may have changed.  Always worth talking to your Spanish bank and local insurance brokers so you can ensure you're getting the correct level of cover for your requirements.
